What is the PHQ-9?
The PHQ-9 is a 9-item self-report tool based on DSM diagnostic criteria for major depressive disorder, developed by Kroenke, Spitzer, and Williams and validated for both screening and tracking treatment response over time. Each item is scored 0-3 based on frequency over the past two weeks.
Interpretation

| 0-4 | Minimal or none |
| 5-9 | Mild |
| 10-14 | Moderate |
| 15-19 | Moderately severe |
| 20-27 | Severe |
Item 9 always matters
Item 9 asks about thoughts of self-harm. A positive response to this item should always prompt a direct, immediate assessment of suicide risk, regardless of the total score — a mild total score does not make a positive item 9 less significant.
